 Commencement of Phase 1b

 

Middle Ridge Family Practice will commence Phase 1b COVID-19 vaccinations on Saturday 27 March 2021 at 1:00pm and Sunday 28 March 2021 from 9:00am.

 

It is recommended that all patients download the consent form from our website and bring this to their appointment.

 

If you are not a patient of this practice and do not have a My Health Record, you will be required to bring a Health Summary from your normal doctor.

 

For all emergency workers, health care workers, carer and disability workers you will be asked to show proof of employment i.e., letter of employment or ID card.

 

Bookings will be available online from 11 March 2021 for all patients who have been allocated to Phase 1b either through HotDocs or through the Commonwealth Booking Platform.

 

Phase 1b

That includes people aged 70 and over, Aboriginal and Torres Strait Islander people over the age of 55, and younger adults with underlying medical conditions or disabilities.

 

General healthcare workers will also be able to access vaccines at this point – and so will Australia’s ‘critical’, ‘high-risk’ workforce, including defence personnel, police officers, firefighters, emergency service workers and meat processing workers.

 

Once Phase 1b has been completed the practice will update this information for Phase 2a.
