Requesting your records

All health records are confidential and are protected under our Privacy Policy. If you are transferring to a different medical practice, you can ask your new practice to send us a signed release for your medical records to be transferred securely.

My Health Record

Please advise if you wish a summary to be uploaded to your My Health Record by our doctors. All Australians had a My Health Record created from 30 January 2019 unless they opted out. For more information on My Health Record, click the button below.
